Emergency intake
For incidents that need DDES support: a plant defect with workers exposed, a near miss that needs assessing, a regulator on site, or a suspected breach. Submitting this form alerts DDES by email. We respond Monday to Friday, 7am–6pm AEST; this is not an emergency service. Forms sent outside those hours are picked up from 7am the next business day.
If anyone is injured or in immediate danger, call 000 first.
If the incident is notifiable, you must notify Workplace Health and Safety Queensland immediately, whether or not you have contacted us. Then submit this form, or call DDES in business hours on 0477 700 825.
